I give permission to officials of my institution to release official transcripts of my academic record and other information requested for consideration in the HITEC Foundation Community Scholarship Program. If selected as a HITEC Foundation Community Scholarship Scholar, I agree to have my information shared with sponsoring corporation for internship and job opportunities. I affirm the information contained herein is true and accurate to the best of my knowledge and belief.
